Osteoporosis and Frailty: What 2026 Research Reveals About Supplements

VITAMIN D AND FRAILTY: WHY STEADY BEATS MEGA-DOSES
Researchers Viola and colleagues explored how vitamin D works in many parts of the body, aiding muscle strength, bone renewal, immune health, and energy production. (1) Low vitamin D levels are connected to muscle loss, weaker bones, and the onset of frailty. However, supplements tend to offer the most benefit when someone is actually deficient or especially vulnerable, and large, infrequent "mega-doses" have been linked with more falls and fractures. The lesson is simple: have your levels checked, and take vitamin D consistently as recommended.
CALCIUM AND VITAMIN D: A FOUNDATION FOR OSTEOPOROSIS CARE
A review by Pascual-Pastor and Barceló-Bru called daily calcium and vitamin D a foundational pillar of osteoporosis care. (2) The authors reviewed the science on how the two nutrients work as a team to cut fracture risk and maintain bone density, and noted that vitamin D deficiency is still widespread even in people who already have osteoporosis. They also highlighted the importance of supplementation for people taking osteoporosis medications.
DO PROBIOTICS HELP BONE DENSITY?
The news on probiotics was less encouraging. In a rigorous trial, Resciniti and colleagues gave 114 women in the first years after menopause either a daily probiotic or a placebo for a full year. (3) The probiotic did not slow bone loss, a valuable reminder that exciting ideas still need scientific support, and that no single pill replaces a personalized plan.
